About the Position:

Community Care is seeking a dedicated CNA to provide skilled visits to our clients. As part of our trusted team, you'll deliver personalized care while enjoying the support of a family-run agency that has proudly served local communities for more than three decades.

Available Shifts:

Shifts Vary

Responsibilities Include:

- Assist clients with activities of daily living (ADLs) including bathing, dressing, grooming, and toileting.

- Provide mobility assistance and transfers.

- Monitor and record vital signs and health status.

- Assist with medication reminders.

- Document care provided in accordance with agency policies.

- Communicate effectively with clients, families, and healthcare team members

Requirements:

- Current and valid CNA certification in Pennsylvania.

- CPR certification.

- Valid driver's license and reliable transportation.

- Ability to work independently and as part of a team.

- Excellent communication and interpersonal skills.

- Previous home health experience preferred but not required
