About the Position:

Community Care is seeking a dedicated RN to provide skilled visits to our clients. As part of our trusted team, you'll deliver personalized care while enjoying the support of a family-run agency that has proudly served local communities for more than three decades.

Available Shifts:

Monday - Friday: 8:00 AM-4:00 PM

Weekends Available

Responsibilities Include:

- Conduct skilled nursing visits in patients’ homes

- Develop and implement individualized care plans

- Educate patients and caregivers on managing conditions and medications

- Collaborate with a multidisciplinary team to ensure continuity of care

- Document visits and patient progress accurately and timely

Requirements:

- Current RN license in Pennsylvania

- At least 1 year of clinical nursing experience (home health experience preferred)

- Valid driver’s license and reliable transportation

- Strong communication, time management, and documentation skills

- Passion for patient-centered care
